STEP 1: Parent/Guardian: Complete Athletic Registration Form (one time per school year for all sports)

STEP 5: Parent/Guardian: Take Physical Examination Form to doctor to complete. Doctor must sign and date form. NOTE: Physical Examination Forms are good for 2 years.
Must have a new physical prior to registering for Fall sports in August of 9th grade year.

STEP 6: Parent/Guardian: If you haven't already, please complete the Family Income Survey and Consent to Share Forms. Parent/guardian must print, sign and date form.

STEP 7: Once you've completed the directions listed in steps 1-6 return all forms (Athletic Registration, Concussion Information Sheet, Medical Emergency Information and Physical Form, Family Income Survey and Consent Form) to Athletics Office. An athletic clearance card will be issued once all forms have been received, all fees have been paid and your student's academic eligibility is verified by the athletic department.

Athletic Eligibility
To be eligible for athletics at Hawkins School a student must:

Live within the boundaries of the North Mason School District and be living with your parents or legal guardian.

Be in full attendance in school last semester and have passed all classes.

Be a full-time student currently within the North Mason School District and passing all classes.

Not have transferred to Hawkins Middle School for athletic reasons.

Have a completed athletic clearance packet on file and be holding a current, valid student body card from Hawkins Middle School.

Not be a professional athlete in the sport you are playing.
-
Abide by the current Hawkins Middle School athletic code.
